I've just completed the Bushmans Cave Route from the border post, having done Tarn Cave earlier this year. There are a few issues that need to be addressed by Ezimvelo:
The "hiking path" is in serious need of some maintenance and is barely discernable for many sections after the 1st 2km - which lure you into a nice false sense of security. Thankfully there are a number of small cairns, some are just 2 rocks on top of each other, which helped tremendously once they were found. It is tough going however with an overgrown path, long grass and brambles in some sections. There were also 4 sections that were littered with red ticks at this time of year (Summer)
Recommendations for anyone doing this route:
Long pants and gators are not only recommended, they are essential.
Check for ticks regularly
Fill up with plenty of water before the final +- 1km ascent to cave as it is thirsty work.
Allow a little more time than you think you'll need as you'll likely need to do some backtracking to find your way. 
Hiking poles or walking sticks are advantageous given the thick terrain and numerous (maybe 7/8) river crossings.
I usually carry a pair of lightweight trainers for sections that require numerous river crossings which helps keep your boots dry.

The Tarn Cave route whilst pretty damn steep in sections, was far more enjoyable and I'd recommend that if you're looking for an easy way to the escarpment which the Bushmans cave route is touted at, opt for the Tarn Cave route rather. Tarn cave is also far nicer with more spectacular views.

I last did this route end of August, and found it the same as its been since I first did it about 10 years ago.
I didn't have any issues with staying on the trail, but it's less overgrown in winter and also having a general idea of where the trail goes helped a lot.

In terms of water, there is normally water available right up to (and higher) than Bushmans cave in August we filled up above the cave before heading up further), and the berg is very wet at the moment

I went up there in April 2025. It is exactly as you said. Same areas so overgrown I lost direction with the Slingsby maps and has it not been for GPS tools, would have lost the track completely and would have needed to do some compass plotting and navigation. The rock stack markers along the way exist, about every kilometre, but some I actually stumbled over because of thick bush. There is no indication from Enzemvelo section that they will do any maintenance on the route. I would not recommend this route for any person unless you take a machete with, in order to hack your way through the jungle. Pitti, because it is an important link to the contour path that split-off to the right further up on the pass, just after the caves to avoid the summit into Lesotho. Also animal life is almost not visible in the thick environment - even if you go stealth. Abundant water with all the river crossings, but the walking / tripping pace for the 12km distance up is extremely slow and makes over-night camping half-way essential ( that is also hacking away a flattish grass spot if you can find one).

Not so sure about camping half way being essential, it took us 2.5 hours to get to Bushmans cave in August.
This trail is very much like almost all berg trails that follow river valleys
edit: it's also only 10km to the cave, and the trail there is much better than the contour path heading north that you're referring to
